Feedback management - Form sub-tab
The form can be sent to the participants as part of a post-event mailing campaign or you can ask them to fill it in directly during the event.
Creating a Feedback form
You can create and edit the feedback form by clicking on the "Feedback" tab and "Form" sub-tab in the left navigation menu.You can modify various fields, preview and finally publish the form via the form settings screen on the right.
Available fields are as follows: (required fields are marked with an asterisk):
- Title (what is the title of the feedback topic being addressed for example: “Event Content”)
- Description (displayed below the Title): Brief description outlining what is being measured for example: “was the event content relevant” or “was the event content clearly presented” or simply ask participants to fill in the form
- Form Fields (content and format of fields used to rate responses)
- Submit button text (text displayed on the submit button)
- Confirmation Message (message displayed in two places: as a confirmation message after the form is submitted and also in the feedback confirmation email that the system automatically sends)
In the "Form Fields" found on the Form sub-tab, you will enter the wording of each question, their order, the specific response display type, and response options.
Note: You can determine the order of the questions by holding the cursor over the entry of a specific question and moving it to the desired order in the list.
Click "Add" button to add a new feedback question by entering the following information (required fields are marked with an asterisk)
- Question text - wording of your question
- Response Style - o
- Free Text
- Check boxes (select one or more options from multiple choices)
- Radio button (select one option from multiple choices)
- Linear scale (select one option from multiple choices on a sliding scale From - To)
- Drop-down (select one option from multiple choices)
- Long text
- Date
- Date and time
Note: For each question, you can choose whether answering specific questions will be mandatory and therefore a condition for submitting the form.
The Feedback form allows you to create conditional based questions (also called conditional logic). A conditional questions form is a dynamic form that changes based on the answers provided by the user. A feedback form, for example, might use conditional logic to display different questions to different users based on their previous responses.
For example, if you want to know from the feedback questionnaire if a respondent organizes events as part of their job so that you can target more relevant “event based” questions to them. First create a question - Do you organize events? Answer option - yes/no. If the user chooses yes, you can ask further - What types of events do you organize? And create the answers - Live, Virtual, Hybrid. Based on their response you can further provide questions specific to the event type such as “who are your video streaming providers of choice in your virtual event?
